Step 4: Select the Authentication Mode as Adhaar.
Step 5: Enter the OTP sent to your registered mobile number and click the “Verify” button.
Step 6: Complete the e-KYC process by clicking your live photograph.
Step 7: Enter all the required details and click the “Submit” button.
Step 8: Your Ayushman Card will appear on the screen. Click on the “Download Card” button and take a printout of the same for further use.

The amended list issued during the second phase of the PMJAY scheme contains about 1574 medical procedures under 24 different categories.
