Mumbai (Maharashtra) [India], July 1: For many women facing fertility challenges, conditions like Polycystic Ovary Syndrome (PCOS) and Premature Ovarian Insufficiency (POI) can make the path to pregnancy complex and uncertain. FSH — produced by the pituitary gland — plays a vital role in the maturation of ovarian follicles, which contain eggs. In PCOS, women typically have a high number of small follicles, but hormonal imbalances can disrupt ovulation. In contrast, POI is marked by diminished ovarian reserve and elevated FSH levels, signalling poor follicle responsiveness.
“By carefully analyzing FSH trends — alongside other markers like AMH (Anti-Müllerian Hormone) and estradiol — we gain a dynamic picture of how a woman’s ovaries will respond to IVF stimulation,” says Dr. Pai.
In women with PCOS, Dr. Pai employs low-dose gonadotropin stimulation protocols combined with precise cycle monitoring. This approach minimizes the risk of Ovarian Hyperstimulation Syndrome (OHSS), a serious complication commonly associated with PCOS. In addition, medications like letrozole may be used to promote more physiological follicular growth.
“PCOS is not an insurmountable barrier,” Dr. Pai emphasizes. “With a gentle, hormone-informed approach, we can safely retrieve high-quality eggs and achieve excellent success rates.”
For patients with POI — often diagnosed before the age of 40 — the challenge is greater. Elevated FSH levels indicate a diminished ovarian reserve, and IVF outcomes can be unpredictable. However, Dr. Pai’s advanced protocols, developed through decades of research, have helped many women with POI achieve pregnancy. These include the use of androgen or growth hormone pre-treatment, individualized stimulation regimens, and, where appropriate, counselling on the option of using donor eggs.
“The goal is always to offer the best path to a healthy baby — whether through a woman’s own eggs or with the assistance of donor programme,” Dr. Pai explains.
One recent patient success story illustrates this personalized, hormone-based approach. A 34-year-old woman with PCOS and multiple failed IVF cycles elsewhere came to Bloom IVF with an AMH of 8 ng/mL and normal FSH, but with irregular ovulation and prior episodes of OHSS. Dr. Pai’s team employed a low-dose antagonist protocol with a GnRH agonist trigger, carefully avoiding OHSS. Fourteen mature oocytes were retrieved, resulting in 10 high-quality embryos. A single euploid embryo was transferred — leading to a successful pregnancy.
In another case, a 37-year-old woman diagnosed with POI and an FSH of 20 mIU/mL had been advised by other clinics to consider egg donation. However, after a comprehensive evaluation by Dr. Pai’s team — including Doppler ultrasound assessment of ovarian blood flow — a tailored stimulation cycle was attempted. Two mature oocytes were retrieved, one high-grade embryo was developed, and the patient is now 10 weeks pregnant following frozen embryo transfer.
“This is why we leave no stone unturned,” says Dr. Pai. “Sometimes, even one good egg can make the difference.”
India faces a growing fertility crisis, with approximately 1 in 6 couples experiencing infertility. PCOS affects an estimated 10–15% of Indian women of reproductive age, while POI is increasingly recognized as an underdiagnosed contributor to infertility. Despite these challenges, Dr. Pai remains optimistic.
